Welcome to the forms section of the Camp Fire USA Balcones Council Web Site. We have many forms available for download. Most of them are in the pdf format and are viewable with the Adobe Acrobat reader.
Registration
To register as a member, club members must complete an annual registration and a liability release. Medical information forms are also required to register as a member of the Backpacking club or the Paddling club. All registrations should be turned in the club leaders unless registering as an at-large member.
Event Registration
Registrations should be mailed with payment or scholarship request to the Camp Fire Office.

Volunteers
Volunteers working with kids must complete the program volunteer application while all others must fill out the general Volunteer Application. An authorization to complete a criminal history check is required for all volunteers. Program Volunteers are required to complete the first page of the Personal Reference response form.
